  • SCID-confirmed diagnosis of schizophrenia or schizoaffective disorder
  • Partially remitted: symptomatic despite antipsychotic benefit, due to inadequate efficacy, inability to tolerate therapeutic dose, or significant side effects
  • At least 2 months of antipsychotic treatment prior to enrollment
  • Regular outpatient mental health services: at least one visit every 3 months for the past 6 months
Key exclusion· 9
  • Severe current symptoms or side effects necessitating urgent medication change
  • Currently taking 3 or more antipsychotic medications for daily use (PRN and mood stabilizers allowed)
  • Current or prior treatment with clozapine
  • Psychiatric hospitalization within the past 3 months
