- ✓Meets DSM-IV criteria for current opioid dependence disorder lasting at least 6 months, confirmed by positive urine opioid test and positive naloxone challenge test if diagnosis unclear

- ✕Pregnancy or breastfeeding
- ✕Active medical illness that may be hazardous, including untreated hypertension, acute hepatitis with SGOT or SGPT >2× normal, unstable diabetes, or chronic organic mental disorder (e.g., AIDS dementia)
- ✕Active psychiatric disorder that may interfere with or make participation hazardous: schizophrenia, bipolar disorder with mania or psychosis, or depressive disorder with suicide risk or ≥1 suicide attempt in past year
- ✕History of allergic reaction to buprenorphine, naloxone, memantine, naltrexone, clonidine, or clonazepam
