  • Opiate dependence, abuse, opioid maintenance treatment, or positive urine screen for opioids
  • DSM-IV dependence on substances other than alcohol, cannabis, nicotine, or caffeine
  • Neurological conditions including epilepsy, history of brain injury, encephalitis, organic brain syndrome, or documented focally abnormal EEG
